For Disablity claim submission you can prepare the documents below:
1. Disablity Claim Form

You can use the claim history page in Emma to find out the status of your last claim. There are 5 (five) types of status: Submitted, On Progress, Approve, Reject, and Pending if your documents are not completed or you uploaded them incorrectly.
No. To submit a Death Claim, you must submit by coming to the nearest AXA Mandiri branch office and make sure to bring complete documents for submitting a death claim.
Emma AXA Mandiri's transaction features are:
1. Single Top Up
2. Switching or Redirection
You can perform Redirection in Emma AXA Mandiri with the following steps:
1. Login to your Emma AXA Mandiri Account
2. Select Switching / Redirection Menu
3. Make sure your Policy is a Unitlink / PAYDI Policy
4. Then select Switching / Redirection and also the required data in the Switching / Redirection Form
5. Select the Fund that you want to Switching / Redirection
6. Select the tax reporting information as per your requirement
7. Upload tax slip (if any)
8. Complete the OTP request process via SMS, and enter the OTP sent via SMS
9. If the OTP is successful, then your transaction will be processed
To see the tracking status/history of your Redirection, please use the Status History feature in the Emma AXA Mandiri menu.
E-Claim is features for submitting health claims through Emma AXA Mandiri.
The maximum limit of E-Claim in Emma AXA Mandiri is IDR 10 Million.
If above IDR 10 Million, you must submit original documents to AXA Mandiri for the purpose of the claim process.
If your documents are incomplete, you can save them as a draft of your claim form. If your documents are complete, you can continue submitting your claim.
You can submit the following types of claims
1. Inpatient
2. Outpatient
3. Critical Illness
4. Disablity
Just by doing a simple registration step, you can enjoy all the advantages and benefits
of Emma AXA Mandiri in the palm of your hand.
1. Click 'Register' on the login page on the https://axa-mandiri.co.id/web/customer or
check through the AXA Mandiri website and click 'Meet Emma'.
2. Enter your policy number and date of birth
3. Enter your mobile number
4. Enter the OTP code you received via sms to your registered mobile number/email
5. Enter and confirm the password for your Emma AXA Mandiri account

One of the requirements to register as a Customer is that you must already have a policy from AXA Mandiri
You can use a Perfect Rewards account to login to Emma because the account used is the same. However, not all customers who have an Emma account can log in to Perfect Rewards because only certain customers can get point rewards
1. Use the “Forgot User Password” menu to reset the password
2. Fill in your User ID, Phone Number and Date of Birth to verify your data

Emma AXA Mandiri e-card consists of 3 (three) categories, where the benefits are adjusted to the choice of products owned by the customer. The three categories are Gold, Platinum and Signature.
a. Gold: AXA Mandiri customers who only have a life insurance policy
b. Platinum: AXA Mandiri customers who have health and/or critical illness insurance policies
c. Signature: AXA Mandiri customers who have health and/or critical illness insurance policies with an annual premium of IDR 100 million
You only need to show the Emma e-card to the merchant who works with AXA Mandiri, in accordance with the terms & conditions stated in the Merchant's Wellness
There are several virtual events or activities, namely Zumba, Yoga, Pilates classes, Chat With Emma (Consultation with a Psychologist) and Webinars about health. Currently, you can take part in various online activities & classes for free, as long as the quota is still available. The availability of these activities & classes is adjusted to the type of Emma e-card owned by the customer.

No, only customers who have Unit link products (PAYDI) can do Top-Up / Switching / Redirection.
The data will be saved automatically if you have pressed the "Save as Draft" button or the "Continue" button. Then, you can continue filling out your Top-Up / Switching / Redirection form

You can make a health claim online from the Claim menu on your Emma AXA Mandiri page and follow the instructions. However, for claims over IDR 10 million, you will still be asked to send all documents in hardcopy to: PT AXA Mandiri Financial Services AXA Tower 8th Floor Jl. Prof Dr Satrio Kav. 18 Kuningan City Jakarta 12940. You can also go directly to the AXA Mandiri Customer Care Center at AXA Tower UG floor at the same address.
You can get Rewards points by adding insurance/buying a new policy, or making payments for your current insurance policy. We need to say that point rewards are one of the customer loyalty programs for AXA Mandiri where there are terms and conditions for those who can get these point rewards.
Make sure your policy and account data are as follows:
1. Make sure you have the same name on your policy with the other policy.
2. Make sure your Emma AXA Mandiri account data is the same as the Policy data you registered. (Example: the mobile number on the policy and the mobile number when registering for an Emma AXA Mandiri account must be the same)
3. Make sure whether your insurance product is registered with Emma AXA Mandiri
If you are a customer or policy owner of AXA Mandiri, then you can access all the features available on Emma. However, if you register as a guest, you will only have limited access.
You can register with Emma as a Guest to try and experience the features contained in Emma AXA Mandiri.

You can register your policy on the Emma AXA Mandiri main menu by:
1. Click 'Connect Policy' on My Page.
2. Fill in your policy number and ID card number.
3. Click 'Connect Policy'.
4. Verify the OTP that goes to the email or SMS.
5. Your policy will be connected.
Besides “Curhat dengan Emma”, currently we do not limit the use of features in Emma AXA Mandiri as long as you have a Gold, Platinum, Signature card, subject to the available classes. However, if in the future there is a change in the feature usage quota, we will immediately inform you again.
Because this feature is face-to-face, we currently limit the use of this feature to 1 customer per week.

The entire activity process from registration to using Emma AXA Mandiri is free of charge at all.
Currently, you can submit a claim through Emma AXA Mandiri, but the withdrawal and policy closing features are currently still not available.

You can Top Up in Emma AXA Mandiri with the following steps:
1. Login to your Emma AXA Mandiri Account
2. Select Top Up Menu
3. Make sure your Policy is a Unitlink / PAYDI Policy
4. Then fill in the Top Up nominal and the required data in the Top Up Form
5. Choose the sub-fund that you want to Top Up
6. Select the tax reporting information according to your needs
7. Upload Overbooking Slip, Tax Slip (if any)
8. Complete the OTP request process via SMS, and enter the OTP sent via SMS
9. If the OTP is successful, then your transaction will be processed
To view your Top Up status/history tracking, please use the Status History feature in the Emma AXA Mandiri menu
You can perform Switching in Emma AXA Mandiri with the following steps:
1. Login to your Emma AXA Mandiri Account
2. Select Switching / Redirection Menu
3. Make sure your Policy is a Unitlink / PAYDI Policy
4. Then select Switching / Redirection and also the required data in the Switching / Redirection Form
5. Select the Fund that you want to Switching / Redirection
6. Select the tax reporting information as per your requirement

Anda perlu menyiapkan dokumen seperti soft copy KTP asli pemegang polis dan penerima manfaat, kartu keluarga asli, akta kematian tertanggung, surat keterangan kematian, dan dokumen lain yang diperlukan. Pastikan semua dokumen yang diperlukan sudah lengkap untuk diunggah sesuai langkah-langkah yang ada di Emma AXA Mandiri.
Ikuti prosedur yang ditentukan untuk mengisi formulir klaim dan unggah dokumen yang diperlukan.

- Jika dokumen yang dibutuhkan adalah mandatory, maka Anda tidak dapat melanjutkan proses klaim meninggal dunia melalui Emma AXA Mandiri. Apabila dokumen yang dibutuhkan tidak mandatory, Anda tetap bisa lanjut untuk pengajuan klaim meninggal dunia melalui Emma AXA Mandiri.
